Qualifications:
Meets the qualifications as established by APTA or other professional organization Licensed to practice Physical Therapy in the current state of practice Must be willing to work in home and clinic-based settings Valid, unexpired Driver's License and automotive insurance Experienced in pediatric rehabilitative services preferred Compensation may vary based on a variety of factors including:
Experience, Education, Productivity, Supervision, Location and other factors.

CCPA Notice for Job Applicants, Contractors, and Employees Residing in California Position Overview The Physical Therapist is responsible for planning, conducting and implementing physical therapy treatment program in accordance with the medical referral and evaluation to restore or maintain patient's wellbeing while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions o Delivery of physical therapy services to comply with prescribed physician orders.
o Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
o Provide a written evaluation defining physical therapy needs, goals, progress and expected outcomes in conjunction with referring physician's Plan of Treatment.
o Participates in coordination of services (including discharge planning) with health care team, physicians, parent/guardians, nurses, etc.
o Maintain effective communication between staff, healthcare team members and family.
o Serve as a resource consultant for clinical staff.
o Supervises other personnel (PTA, HHA) as applicable.
o Completes OASIS assessments where allowed by state professional practice and regulation.
Requirements o Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program o Current, unrestricted state license as a Physical Therapist in the state of practice o Current CPR certification o Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures Additional state
Specific Requirements:
